E-Sign Consent and Disclosure Form:
This E-Sign Consent and Disclosure Form (“Consent Form”) applies to all communications for any account that you may receive from C. Tech regarding your account(s) that have been placed for collection with C. Tech, except as otherwise prohibited by law.
Please read this Consent Form carefully and in its entirety. This Consent Form affirms your consent to the electronic delivery of any disclosures, agreements, documents, notices, or other notifications, including those required by applicable law (collectively “Documents”). A copy of this Consent Form will be e-mailed to you at the e-mail address you provide below.
Even after signing this Consent Form, you have the right to request a paper (non-electronic) copy of Documents, however, C. Tech may not provide you with paper copies of any Documents unless specifically requested by you. You will not be charged a fee for any paper copies requested.
Please note that agreement to this Consent Form will not result in enrollment of paperless statements as C. Tech does not send regular statements on any account.
Definition
The words “we”, “us”, and “our” refer to C. Tech Collections, Inc., and the words “you” and “your” refer to you, the consumer responsible for the account(s) placed in our office and your authorized agents.
“Account” means the account(s) for which you are financially responsible and obligated to pay that have been placed with C. Tech for collection.
“Electronic signature” means symbols or other data in digital form attached to an electronically transmitted document as verification of the sender’s intent to sign the document.
Communications to be Provided in Electronic Form:
By consenting to this agreement, you agree that C. Tech may provide you with any communications that we may choose to make available in electronic format, to the extent provided by law, and that we may discontinue sending paper communications to you, unless and until you withdraw your consent as described below. Your consent to receive electronic communications includes, but is not limited to:
- Any communication concerning the status of your account(s) that have been placed with C. Tech for collection;
- Any legal and regulatory disclosures and communications associated with your account(s), unless otherwise prohibited by law from being sent electronically;
- Communications related to a change in the terms of your account(s) or any associated payment arrangements;
- Privacy policies and notices.
We also need your consent to use electronic signatures in our relationship with you.
Method of Providing Communications to You in Electronic Form:
All communications that we provide to you in electronic form will be provided either:
- via e-mail;
- by access to a website that we will designate in an e-mail notice that we sent to you at the time the information is available;
- to the extent permissible by law, by access to a website that we will generally designate in advance for such purpose; or
- by requesting you download a PDF file containing the communication.
How to Withdraw Consent:
You may withdraw your consent to electronic communications at any time by:

You have the right to withdraw your consent to delivery of all communications in electronic form for any of your accounts, which may include payment arrangements, payment reminders and notifications, account status and information, terms and conditions, notices, disclosures and any and all other documents. Withdrawal of consent will apply to all electronic communications, including payment arrangements, payment reminders and notifications, account status and information, terms and conditions, notices, disclosures and any and all other documents.
At our option, we may treat your provision of an invalid e-mail address, or the subsequent malfunction of a previously valid e-mail address, as a withdrawal of your consent to receive electronic communications. You will not be charged any fee to process the withdrawal of your consent to receive electronic communications. Any withdrawal of your consent to receive electronic communications will be effective only after we have had a reasonable period of time to act on your notice of withdrawal. Once you have withdrawn consent, you may only be allowed to receive electronic communications in the future by re-signing and re-submitting this Consent Form.
How to Update Your Contact Information:
It is your responsibility to provide us with a true, accurate, and complete e-mail address and other contact information. It is also your responsibility to provides us, promptly, with any updates to your contact information. You may provide updated contact information to us by:

Communications in Writing:
All communications that we sent to you in either electronic or paper format will be considered to be communications “in writing.” We recommend printing, downloading, or otherwise saving copies of all such communications, including this Consent Form, for your personal records. A copy of this Consent Form will be e-mailed to the e-mail address you provided.
Federal Law:
You further acknowledge and agree that your consent to electronic communications is being provided in connection with a transaction affecting interstate commerce that is subject to the E-SIGN Act, and that the E-SIGN Act shall apply to the fullest extent possible to validate our ability to conduct business with you via electronic communications.
System Requirements to Access Information:
To access, receive, and sign Documents, you must have the following equipment/hardware and software:
- A personal computer, tablet, or other device that is capable of accessing the internet;
- Software that is capable of displaying PDF files;
- An Internet connection;
- An active e-mail address;
- An Internet web browser capable of supporting 128-bit SSL encrypted communications and your systems or device must have 128-bit SSL encryption software.
To the extent that you choose to access electronic communications from us on your mobile device, data charges through your cellular service provider may apply.
If our hardware or software requirements change, and that change would create a material risk that you would not be able to access or retain your Documents, we will give you notice of the revised hardware or software requirements. Continuing to use electronic communications after receiving notice of the change in hardware and software requirements is reaffirmation of your consent.
Communication in Languages Other Than English – New York City Residents
C. Tech does not conduct business in any language other than English. C. Tech does not provide any language access or translation services for any language other than English. A translation and description of commonly used debt collection terms is available in multiple languages at www.nyc.gov/dca
Consent & Electronic Signature:
By entering your information below, signing, and returning a copy of this Consent Form to us, you are providing us with your affirmative consent to use electronic signatures and to use and receive electronic records and communications from us. By providing your consent, you further agree that your computer system (or other system you will use to receive electronic communications from us) satisfies the hardware and software requirements specified in the above section titled “System Requirements to Access Information” and that you have provided us with your current and valid e-mail address so that we may send you electronic communications.
